Caregivers

Our caregiver teams include highly-screened Case Managers, Registered Nurses, Licensed Practical Nurses, Certified Nurse Aides, Home Health Aides, and Personal Care Attendants.

Each individual offers impeccable credentials and receives specialized training in order to provide care that addresses the medical, cultural, psychological, emotional and spiritual dimensions of our clients’ lives and conditions.

Administration

From Branch Managers and Program Coordinators to Registered Nurse Client Case Managers, our administrative and management staff supports our clients and their caregivers to maximize the effectiveness of our care.

We consult with our clients’ physicians, social workers, hospitals, other providers and families to develop an individualized plan of in-home care. We also handle the necessary paperwork, working with providers, insurers and government agencies as needed to establish and maintain each client’s services.

Leadership Team

Tim Lively, Founder and President

First Nations Home Health founder Tim Lively holds a Bachelor of Arts degree in Marketing, with a Minor in Finance, from the University of St. Thomas.

Tim served on the Board of Directors of Parenting with Purpose, has worked with microfinance initiatives in India, and studied models of community health in Haiti in order to translate key strategies and tactics to his work in the Midwest and Southwest. Tim lives in Maplewood with his wife Michelle and their three children.

Working alongside the President to ensure competent and compassionate care delivery is a world-class leadership team of professionals that includes community leaders, Regional Directors, a Director of Patient Care, a Director of Quality Assurance/Compliance, a Human Resources Manager, Sagrado Corazón’s Controller, and Consulting Advisors.
