First Nations serves our Native American population through our Duluth, Bemidji and Minneapolis offices.

We understand how overwhelming and difficult it can be to care for an elder loved one at home. First Nations Home Health serves the Native American population through seven of our branch locations in the Minneapolis-St. Paul metro area, and greater Minnesota, including the Duluth and Bemidji areas. With an approach that is culturally-sensitive to the unique ethnic and linguistic needs of Native Americans, our services are specifically designed to enhance the health and well-being of individuals both on and off the reservation in Red Lake, White Earth, Leech Lake, Bois Fort, Fond du Lac and Mille Lacs. Through our Duluth office and Bemidji area locations we offer a variety of home health care services to help support your loved one to live a more independent and fulfilling life at home, whether they’re in an urban or rural setting.

Our team of caregivers, nurses and case managers (many of whom are Native American) provide services that help elders stay at home in familiar cultural surroundings, develop good health habits and reduce hospitalizations. With services such as skilled nursing, mental health, personal care and homemaking, at First Nations Home Health, our goal is to improve the health status of Native Americans who experience an unusual high rate of preventable acute and chronic illnesses.
